Moving through the challenges of palliative and end of life care is made easier with support from compassionate communities.

Primary Health Tasmania has been successful in receiving funding to implement the Australian Government funded Greater Choice for At Home Palliative Care Measure (the measure) in Tasmania from July 2018 to June 2019.

The aim of the measure is to improve the coordination and integration of end of life care across the primary, secondary, tertiary and community health service to support at home palliative care. The aim of the Tasmania project, informed in consultation with the Partners in Palliative Care Reference Group, is to effectively work with identified Tasmania rural communities to design and implement team-based care approaches to improve access to end of life care in local communities.

Primary Health Tasmania is now working with the Tasmanian Greater Choice Working Group to identify communities who are ready to undertake a place-based approach within a compassionate community framework.

    · At the Derwent Valley community meeting held on 1 May 2019 it was decided not to pursue the formation of a consultative group (as per Glamorgan-Spring Bay).

    · Primary Health Tasmania hosted a National Palliative Care Week event at Library Tasmania New Norfolk.

    · In late May/early June stakeholder research revealed the Circular Head and Waratah-Wynyard councils work closely together, recently launching a Community Health & Wellbeing Plan 2019-2024. This resulted in the Tasmanian Greater Choice Working Group agreeing to pursue this region to next implement the Greater Choice measure. Christine Materia spent several weeks consulting with palliative care service providers, GPs, council elected members, staff, community groups and non-government organisations.

    · Expressions of interest later went out to the community for the role of community coordinator for the Circular Head/Waratah-Wynyard region. Lynne Jarvis will begin this role on the 8 July. Lynne has an extensive background in health and community services, including a position with Palliative Care Tasmania and The District Nurses hospice@HOME program.

    · The Greater Choice Community Consultative Group for Glamorgan-Spring Bay met for the first time in early April.

    · Also in April, Alison Collier, a former palliative care specialist and oncology nurse, with extensive experience in the United Kingdom and Melbourne, was appointed to the role of community coordinator. Alison has been busy establishing networks across the region and is facilitating strong community connections.

    · As part of National Palliative Care Week in May, events were held in Swansea, Bicheno and Triabunna. Members of the consultative group attended the Dying To Talk Expo in Launceston.

    The Glamorgan-Spring Bay (GSB) Greater Choice Consultative Group’s first meeting was in April 2019 and has met once since. A Greater Choice Community Coordinator has been appointed. Together they will work with communities to:

    • strengthen community connections
    • improve knowledge of palliative care and end of life care
    • promote available services
    • support conversations around palliative care, death and dying.

    As part of National Palliative Care Week (19-25 May 2019) the GSB Greater Choice Consultative Group and the Community Coordinator are holding events to start conversations to bust palliative care myths. Topic areas are: ‘Let’s Start Talking’, ‘What Matters Most’ and ‘What Choices You Have’. The locations, dates and times for each event are as follows:

    • Triabunna at The Village Market on Sunday 9 May 2019 from 10am to 1pm
    • Swansea at The General Store on Monday 20 May 2019 from 10am to 2pm
    • Bicheno at IGA on Wednesday 22 May 2019 from 10am to 2pm
    • Launceston at The Dying to Talk Expo on Saturday 25 May 2019 - this is a full day trip. A community car trip/car pool is available. Contact the Community Coordinator for more details. Also for more information about the expo visit the website www.dyingtotalkexpo.com.au/
